The different disciplines within a facility work as a team. If a member of the team is not following safe medication practice or is engaged in drug diversion, the whole team is at risk. Each staff member is responsible for following policies and procedures. The next team member takes over and works on the premise that the previous shift followed the policies. If they didn’t, something may get missed and leave the oncoming shift vulnerable to making an error. If one of the team members has a substance abuse problem and is involved in diversion, they are the most at risk and need immediate help. The healthcare team needs to have the understanding and tools to address all medication safety and diversion concerns timely.
